

Diabetes Management Supports
Diabetes management supports are what the NDIS call "disability-related health supports". This support can only be funded if they relate to your disability and help you undertake activities involved in day-to-day life.

Diabetes Management Plan
If you are unable to manage your diabetes independently due to your disability and it meets the NDIS funding criteria, the NDIS may provide funding for a nurse to develop a participant specific Diabetes Management Plan for you.
This funding covers an initial consultation, development of the plan and re/assessment.

Training
We offer contextualised training for disability support workers and family to help them understand and effectively manage your diabetes. This training ensures that disability support workers are equipped with the knowledge and skills to assist with daily diabetes management, including:

Monitoring blood glucose levels

Administering insulin or other diabetes medications

Recognising and responding to signs of hypoglycaemia or hyperglycaemia

Supporting dietary and lifestyle needs related to diabetes
