

Wound and Pressure Care Supports
Wound and pressure care supports can include funding to help develop and carry out a wound and pressure care plan. They can include equipment to prevent pressure areas or wounds occurring, or products to help manage wounds.

Wound Prevention Plans
A Wound Prevention Plan aims to reduce the risk of wounds, particularly pressure injuries, in participants who may be at higher risk due to factors like immobility and incontinence. The plan focuses on proactive measures to maintain skin integrity, prevent complications, and improve overall quality of life.

Pressure Care Plans
A Pressure Care Plan aims to prevent and manage pressure injuries in participants at risk due to factors like immobility and incontinence. The plan includes specific strategies to maintain skin integrity, alleviate pressure on at-risk areas, and support healing for existing pressure injuries. It also helps guide funding for essential supports, such as pressure-relieving devices.

Wound Management Plans
A Wound Management Plan outlines the care and treatment of a participant’s wound/s. It ensures that the participant receives safe, consistent, and effective wound care while promoting healing and preventing complications. The plan is tailored to the individual's specific needs and considers their disability-related health conditions, level of support, and their environment.

Training
We provide training for disability support workers and family to assist with the implementation of a Wound Management Plan or Pressure Care Plan, ensuring they have the necessary skills and knowledge to support the participant’s care needs. As per the delegation of care model (NDIS), this training and oversight by a nurse are appropriate to ensure safe and effective care.
